NEW PATIENT INSTRUCTIONS

Thank you for your interest in Bay Area Committed to Kids Medical Group. We look forward to being able to help you and your family. To help us prepare for your first visit, please read the following instructions. We have found that a mutual understanding from the beginning of service of how our program works helps ensure that you and your family will be successful in achieving the goals you are motivated to achieve.

Prior to your first appointment, we ask that you fill out and return the forms found in your initial packet (availability to be downloaded from our webpage at www.bactk.com coming soon!). The information allows us to more thoroughly get to know you and your family while maximizing the time spent with our health professionals on your first visit. Please read the questions carefully and try to be as thorough as possible.

An "Authorization to Release Information" form is included in your initial packet.

It is helpful for us to have past medical records, including growth charts, consultation notes from other healthcare providers, and previous lab work prior to your first visit. Please send or give this to your primary medical provider prior to your first visit, as time allows.


WHAT TO EXPECT

We will want to assess you over a series of appointments during which time you will meet many, if not all, the health professionals that comprise our multi-disciplinary program. During the first visit (1.5 hrs) you will meet with Dr. Jamie (Wallach), our board-certified Physician Nutrition Specialist and Pediatrician to discuss and clarify the issues that you think you want help with.  We will extensively review your child's medical and developmental history, family medical history, and school history (as applicable). At that visit, you will determine with Dr. Jamie the subsequent appointments in an order that best fits your family and child's need.  Those visits will be hour long visits with the nutritionist, behavior specialist, and pediatric fitness specialist.


YOUR COMMITMENT

After focusing exclusively on helping families effectively find a healthy and balanced lifestyle, we have learned that to provide our service successfully, families need to be informed and better understand the commitment involved.

Changing habits, especially those that involve eating and activity, require a level of dedication and commitment that can be sustained through the months and years. We commit to you that we will help you find your family's way. There are general suggestions and recommendations that may fit most families, but we are devoted to helping you adjust your family's current lifestyle only as much as you need to achieve a healthy and balanced way of life.

Ideally, visits will be frequent (weekly or every other week) for the first month or two.  This will help us get to know you better to customize our suggestions, as well as help YOU remain as motivated and accountable as you do when you come to your first visit.  Life has a "funny way" of taking over and "getting in the way" of what we really want. Being accountable to someone else really works. Our goal is to help you be accountable to yourself so we'll reassess often (almost every visit!!) the benefit and reality of your current frequency of visits.  Over the past 4-5 years, we have been successful obtaining insurance reimbursement for all of our services for PPO patients.

Our primary mission is to help children and families with weight problems successfully alter and adapt behavior patterns compatible with improved health, weight, and well-being in the challenging and complicated society we currently live in.

We make it our primary objective to achieve our mission with recommendations and strategies that are medically-monitored and delivered by a compliment of multi-disciplinary health professionals in a comprehensive, compassionate, successful, and sustainable model compatible with improved health, weight, and well-being for all.

 

The Philosophy Behind Our Program

Our philosophy originates from our belief that a supportive, educational, safe, and effective nutrition and fitness program can be designed for anyone.We are committed to customizing known strategies that work to create a program designed specifically to fit you, your child, and your family. We will help you choose and develop healthy lifestyle eating and exercise patterns that will increase self-confidence and self-esteem while helping raise body awareness and promote healthy lifestyle changes.

 

How do we do this?

Our assessment and evaluation session is extensive. We want to get to know you and your family, your habits and struggles as well as what might already be working in your life; what brings you satisfaction and contentment. Our challenge is to understand where you are and meet you there. We just can't slap an intervention (otherwise known as a "silver bullet"‚ "cure"‚ "quick fix") on you. We need to help you identify what do you really want and what are you willing to do.

Why are we successful?

  1. We are continually assessing and refining our program recognizing that time and experience yield better insight and new information as to what is really helpful and has a high probability of working and what does not.
  2. Our program is based on evidence-based medicine practiced and published throughout the United States, as well as internationally.
  3. Our practice is a specialty practice, focusing exclusively on the disease of obesity and related co-morbidities, weight management and medical nutrition. We have acquired the specialized knowledge and experience to treat metabolic and weight problems, which results in a higher standard of medicine in this specific area than is available from the majority of primary care health professionals.
  4. Our reputation in the community is strong, and we have accumulated a wealth of continued information and learning from all those who have come before you.
  5. We are personality-based.  We care and really want to know you and help you. Every child and family is unique and we really like that!
